Someone on Reddit reported this bug last week but I've come across it now too, twice.

When playing the campaign and finishing a level, if you DON'T activate one of the exit points before quiting (or activate one and then quit before it loads) the exit points will be gone when you load the campaign again. This means you are now stuck on that level.
